I hereby request and consent to the performance of acupuncture treatment, massage treatment, manual therapy, and other related procedures, including physical examinations, the use of acupuncture needles and modalities of heat (moxibustion), massage on myself (or the patient named below, for whom I am legally responsible).
Though treatments and therapies are usually beneficial and seldom cause any problems at all, I understand and am informed that as with any medical treatments there are risks involved.
Additional Info
Please bear in mind that you’ll have to go into your underwear to do the treatment. As I will have to do acupuncture and light treatment directly onto your skin. You could bring your own clothes, we would recommend an oversized T-shirt & yoga pants.
Please do not put any lotions or creams on your body on a treatment day.
Please do not eat 30mins before the treatment begins. Please do not book your first appointment when you are on your period unless your condition is causing you unbearable pain and treatment is needed urgently.
